These Terms of Service ("Terms") govern your access to and use of EasyDocForms ("Service"), operated by EasyDocForms ("we", "us", or "our"). By using our Service, you agree to be bound by these Terms.
1. Description of Service
EasyDocForms provides a HIPAA-compliant patient intake form platform that allows healthcare providers to create, distribute, and manage digital intake forms.
2. Eligibility
You must be a licensed healthcare provider, practice administrator, or authorized representative of a healthcare organization to use this Service. You must be at least 18 years old.
3. Account Responsibilities
- You are responsible for maintaining the confidentiality of your account credentials
- You are responsible for all activities that occur under your account
- You must notify us immediately of any unauthorized access
- You must provide accurate and complete information
4. HIPAA Compliance
We will enter into a Business Associate Agreement (BAA) with you as required by HIPAA. You remain responsible for ensuring your use of the Service complies with HIPAA and other applicable laws.
5. Acceptable Use
You agree not to:
- Use the Service for any unlawful purpose
- Attempt to gain unauthorized access to any part of the Service
- Interfere with or disrupt the Service
- Transmit any viruses or malicious code
6. Payment Terms
Subscription fees are billed monthly or annually as selected. All fees are non-refundable except as required by law. We reserve the right to change pricing with 30 days notice.
7. Termination
You may cancel your subscription at any time. We may suspend or terminate your account for violation of these Terms. Upon termination, you may request export of your data within 30 days.
8. Limitation of Liability
To the maximum extent permitted by law, EasyDocForms shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of the Service.
9. Changes to Terms
We may modify these Terms at any time. We will notify you of material changes via email or through the Service. Continued use after changes constitutes acceptance.
